Only use this form if you have already registered a team and paid the registration fee but did not have your team roster at the time.
Eligibility
Players are only allowed to play on one 5-on-5 team and one 3-on-3 team. Younger players can “play up” in any division classification.
Deadline
Submit your roster before midnight on July 1 for your team to be drawn in the brackets.
